ESSENTIAL FUNCTIONS
- Works using guidelines established by the Oregon State Board of Nursing, Nurse Practice Act, Oregon Administrative Rules, Policies & Procedures, and clinical nursing judgment
- Performs comprehensive and focused nursing assessments
- Ensures residents’ individualized service plans address patient needs, updating PRN
- Coordinates care with outside providers, such as PCPs, Home Health, or Hospice
- Performs weekly and PRN skin checks
- Reviews weight variances for clinical factors and development of the service plan
- Delegates applicable tasks to unlicensed staff adhering to all OSBN rules
- Reviews all supportive devices with restraining qualities for resident appropriateness
- Completes documentation thoroughly, accurately, and timely
- Delivers direct nursing care to patients PRN
- Provides clinical oversight to all unlicensed staff
- Initiates and oversees training for staff and residents
- Serves as a leader and role model for resident care
- Adjusts schedule to meet facility needs for training and delegation on all shifts
- Performs all intramuscular injections
- Coordinates care with the interdisciplinary team
- Adheres to all policies, procedures, rules, and regulations
- Audits compliance with policies, procedures, and regulatory guidelines
- Participates in survey processes
- Oversees all narcotic destruction and disposal
- Serves as a resource for the local community
MINIMUM QUALIFICATIONS
- Current Oregon Registered Nurse license without restrictions
- Basic Life Support Certification
- Continuing education per OSBN requirements
- Completion of the “Role of the RN in Community-Based Care” within 6 months of hire
- Experience in a community-based care setting is preferred

Experience:
- Senior Living Facility: 1 year (Preferred)
- Oregon RN License: 1 year (Required)
License or certification:
- Registered Nurse (RN) (Required)
